Description
The United States Air Force is soliciting licensed mortuary services for authorized personnel, including embalming, restoration, preparation, care, casketing, disposition, transportation, death certificates, caskets, urns, outer shipping containers, and related professional services and supplies. The contractor must provide licensed facilities, licensed funeral service practitioners, suitable vehicles, secure and sanitary preparation and storage areas, required equipment, and compliant hygienic practices. Funeral home facilities must be located no more than 50 miles from Little Rock Air Force Base. The resulting blanket purchase agreement has a five-year period of performance and an estimated total value of $250,000, with an estimated annual value of $50,000. Individual calls generally may not exceed $25,000 without contracting personnel approval. Offers must include completed pricing on the provided price list and a technical submission containing a capability statement, the distance from the base to the funeral home, company certifications, and past-performance information. Past performance is limited to four references, with each reference summary limited to two pages and work required to be ongoing or completed within the previous three years.
