Description
Allen County Surveyor's Office is seeking qualified contractors to perform drain maintenance on regulated drains. Services include clearing and grubbing, debrushing, ditch dipping/sediment removal, seeding and restoration, logjam/obstruction removal, and eroded bank repair. Contractors must have the required documents on file with the Surveyor's Office, including a Bid Work Survey, Spec Book Acknowledgment, Certificate of Liability Insurance, Worker's Compensation Exemption Certificate (if applicable), Indiana Legal Employment Declaration, Statement of Company Equal Employment Opportunity Policy, Non-Collusion Affidavit, and Non-Debarment Affidavit. For spraying and debrushing jobs, a Spray License with Category 6 endorsement is required, or a Spray Agreement for Subcontractor must be submitted.
