Description
The Bureau of Indian Affairs is soliciting a lump-sum quote from an Indian Small Business Economic Enterprise for demolition and replacement of concrete walkways and entrances at Administration Building 2. The scope also includes concrete curbs and gutters, landings, a parking-lot island, and a ramp. Work includes furnishing all labor, materials, equipment, tools, services, and insurance; locating underground utilities; removing and disposing of existing concrete; installing replacement concrete at a minimum six-inch depth; providing broom-finished, slip-resistant walking surfaces; maintaining site safety and security; and disposing of project waste offsite. The contractor must provide a detailed work plan, emergency response plan, safety program, respiratory protection program, training/accreditation/qualification certificates, employee list, proof of insurance, chemical safety data sheets, respirator fit-test records, medical surveillance records, HEPA filtration equipment specifications, and copies of regulatory notifications. The offer must include pricing, completed Section K representations and certifications, and the Indian Affairs Indian Economic Enterprise Representation Form. Award will be based on the lowest total evaluated price. The period of performance is 60 calendar days after issuance of the notice to proceed, and the work carries a 12-month workmanship warranty.
