Description
Midwestern State University is soliciting proposals for surface improvements to prevent drainage migration into the tunnel near niches D and E. The scope is defined by the drawings and statement of work in Appendix F and includes existing-condition work, demolition, proposed drainage improvements, and construction details. Respondents must provide labor and materials, pricing, a project supply schedule and lead time, and documentation demonstrating knowledge of local building codes. The response package must include the completed pricing sheet, three current customer references, signed affidavit, addenda checklist if applicable, liability insurance certificate, HUB Subcontracting Plan if the response is at least $100,000, W-9 if applicable, conflict-of-interest affirmation, local building-code documentation, applicable bid and payment bonds, and the completed response packet. The response is limited to 30 pages and must be typed in 12-point font with sequential page numbering. A bid bond of 5 percent is required for responses of $25,000 or more; a payment bond is required at that threshold, and a performance bond is required for responses of $100,000 or more.
