Description
The New York State Office of Mental Health seeks one eligible not-for-profit organization with 501(c) incorporation to establish and operate an Older Adult Technical Assistance Center supporting the Teams to Promote Aging in Place in OMH Supportive Housing grant. The center will provide programmatic and fiscal technical assistance for planning, implementation, operation, evaluation, quality improvement, sustainability, reporting, and cross-disciplinary training related to geriatric service demonstration programs serving older adults and certain younger residents with complex medical needs. Required activities include regular and as-needed consultation; staff training; webinars, distance learning, coaching calls, individualized consultation, in-person site visits, and annual learning collaboratives; assistance with interdisciplinary teams, community partnerships, peer-professional recruitment and supervision, behavioral health and aging services, home care, cultural and language access, technology, data collection, evaluation, and fiscal sustainability. The contract will provide one award of $200,000 annually for five years, subject to available appropriations. Applicants must submit a proposal narrative and five-year operating budget through the Statewide Financial System, with the proposal template and supporting attachments combined into one PDF and the budget submitted as an Excel file. All not-for-profit applicants must be prequalified by the submission deadline.
