Description
Humble Independent School District is establishing an approved vendor list for athletic equipment and supplies, physical education equipment and supplies, first-aid and trainer equipment and supplies, and other related products. Examples include equipment for baseball, basketball, football, golf, soccer, swimming, tennis, track, volleyball, wrestling, gymnastics, jump ropes, balls, disc golf, parachutes, bandages, rehabilitation supplies, treatment tables, and wraps. Turf and playground equipment are excluded. The District is also requesting pricing information, discounts, product catalogs or brochures, product and service descriptions, shipping policies, minimum-order information, reference codes, and responses regarding installation availability, payment methods, and purchase-order acceptance. Awards may be made to one or multiple vendors on a recurring, as-needed basis; placement on the approved vendor list does not guarantee purchases or sales volume. Vendors must submit three to five school-district or higher-education customer references, Form 1295, Form W-9, required electronic certifications and acknowledgments, and any applicable catalog or pricing materials. Installation and other on-site services may trigger background-check and insurance requirements.
