Description
The City of Phoenix Water Services Department is seeking a qualified contractor to provide annual inspections, annual load testing, certification, and as-needed repair services for anchor sockets, associated structural components, and related control systems at water treatment, wastewater treatment, water reclamation, and lift-station facilities. Work must be performed by certified Qualified Persons in accordance with OSHA 1910.140, applicable ANSI Z359 standards, and all applicable laws and regulations. The contractor must furnish labor, equipment, tools, materials, and transportation; maintain inspection and load-test records; provide reports and certification documentation; and correct deficiencies within three business days of written notification. Anchor points must support at least 5,000 pounds per person tied off. Repairs estimated above $150 require a written quote and facility-supervisor approval before work begins. The contract term is five years with no extension options, and pricing includes anchor-point inspection/testing/certification, standard, premium, and emergency labor hours, plus a discount for replacement parts.
