Description
The New York State Department of Health is offering grants through the Rural Community Health Integration initiative to foster collaborative approaches to rural health integration. The program aims to enhance healthcare delivery, coordination, and outcomes in rural areas by addressing health and social needs. Applicants must demonstrate a community-focused approach, including a Lead Applicant, Partner Organization(s), and at least one General Hospital or Rural Emergency Hospital. The Lead Applicant must be a not-for-profit 501(c)(3) organization or municipal hospital with a three-year history of regulatory compliance, located in eligible rural counties. Funded projects must be completed by June 30, 2027. Applicants must address at least one key area: increased access to care, decreased readmissions, or increased preventative care. Eligible Lead Applicants and Partner Organizations include Local Health Departments, Hospitals, Federally Qualified Health Centers (FQHC), Certified Community Behavioral Health Clinics (CCBHC), Rural Health Networks, and various other healthcare providers. The minimum geographic unit for any application is the county, with multi-county applications encouraged.
