Description
The Mississippi State Department of Health is offering up to $30,000 in FY26 FLEX subgrant funding for eligible non-federal entities to support population health capacity and measurable improvements in community health outcomes at critical access hospitals. Proposed activities should address the project objectives and include a work plan, outcome measures, evaluation activities, and a budget. Payments are tied to performance metrics, and costs must be allowable direct or indirect costs incurred during the stated performance period. The selected subgrantee must coordinate communications with the Mississippi Office of Rural Health FLEX Coordinator and Director, acknowledge the funding source in relevant materials, provide activity-related training records and materials, survey participating hospitals, submit quarterly email updates, and provide a final report. Applications are submitted through the department's OpenGov Procurement portal.
