Description
The Defense Logistics Agency is soliciting one restorative dental kit, NSN 6520-01-740-0358, described as an IPS Style Ceram starter kit and metal-ceramic veneering system with oxyapatite crystals. The kit includes ceramic paste opaquer, dentin, deep dentin, incisal, translucent, mamelon, occlusal dentin, Opal Effect, liquids, shade guides, IPS Ivocolor assortment and related glaze and mixing materials. The item must have a 60-month non-extendable shelf life and comply with applicable DLA packaging, marking, hazardous-material, medical-device, FDA, and domestic-material requirements. The quote must identify the source and part number being supplied; alternate offers require a complete data package for evaluation. Delivery is required 20 days after order, FOB destination, with quantity one.
