Description
The State of Montana, Department of Military Affairs, Construction and Facilities Management Office (CFMO) is soliciting bids for cross-connection control plan services for drinking water infrastructure. The scope of work includes developing a Cross-Connection Control Plan (CCC Plan) for the Fort Harrison Military Complex, conducting surveys for various MTARNG facilities across five regions, and delivering draft and final CCC Plans. Bidders must acknowledge and comply with several prerequisites, including the transition from DUNS to Unique Entity Identifier (UEI) on SAM.gov, and ensure their profile is updated. Bidders must also provide their Federal Tax I.D. and UEI. Additionally, bidders need to supply contact information, including name, address, phone number, and email, and complete the 'Covered Telecommunications Equipment or Services Representation' form. The submission requires acknowledging the State of Montana contract terms and conditions, and any requests for additions or exceptions must be submitted to the Q&A board by the closing deadline. Bidders must also acknowledge and comply with Montana Prevailing Wage Rate Requirements for Non-Construction, Montana Time Zone information, and understand that submitting a response indicates acceptance of all terms.
