Description
The City of Carrollton is soliciting sealed bids for the supply and delivery of flexible base aggregate, crushed concrete, and rip rap stone. Bid lines request 11,700 tons of Type D Grade 1 crushed concrete flexible base, 3,200 tons of Type A Grade 1 crushed concrete flexible base, 1,000 tons of 18-by-24 rip rap, and 1,000 tons of 4-by-8 rip rap (gabion stone), along with pricing for delivery time and mileage from the supplying plant to the Central Service Center. Prices must include material hauling and delivery, and deliveries may be required at multiple locations within the city. Bidders must have performed similar work within the past three years and provide at least three references for services performed within the last year. The solicitation requires completion of the electronic bid-event fields and attached forms, including a W-9 and Conflict of Interest Questionnaire, with conditional submissions for HUB certification, litigation, contract defaults or noncompletion, contract terminations, alternate specifications, and additional notes. An original ACORD certificate or state-approved insurance form is required at bid time, with specified commercial general liability, automobile liability, workers’ compensation, employers’ liability, and umbrella or excess liability coverage. The contract is described as renewable, but its base and renewal durations remain placeholders.
