Description
Southwest Independent School District is seeking multiple qualified vendors to provide licensed physical therapy services on an as-needed, non-exclusive basis. Services will support students with physical disabilities and include assessment, eligibility determinations, direct and consultative therapy aligned with Individualized Education Programs, progress evaluation, participation in Admission, Review, and Dismissal committees, family and school-staff consultation, specialized-equipment guidance, documentation, and related professional development. Assigned physical therapists must hold a degree in physical therapy, be licensed in Texas, and have at least one year of experience. The work requires daily district-wide travel, direct student contact, possible lifting and transferring of students, and compliance with criminal-history screening requirements. The district expects to award multiple vendors and use them as needed for vacancies, leave coverage, additional services, or other staffing needs; no minimum or maximum quantity is guaranteed. Responses require firm information, service history, personnel qualifications and licenses, samples, references, capacity and approach narratives, pricing, insurance evidence, and applicable certifications and disclosures.
