Description
The Department of Veterans Affairs is soliciting a certified medical physics team to provide on-site diagnostic medical physics and radiation safety support for the Finger Lakes Health VA Radiation Safety Program. The scope includes annual, initial, and emergent inspections and quality-control testing for radiographic, fluoroscopic, computed radiography, digital radiography, CT, dental, and medical display equipment; shielding design calculations and surveys; radiation safety consultation; quality assurance program support; incident review; training; and written reports. The contractor must provide at least one primary and two alternate certified health or medical health physicists. Key personnel must meet specified certification, education, experience, licensing, background-check, training, and competency requirements. The quotation must include technical capability information, three recent and relevant past-performance references, pricing for all base and option-period line items, required certifications and information-security materials, insurance certificates, and the completed solicitation attachments. The contract is a firm-fixed-price, indefinite-quantity arrangement with a one-year base period and four one-year options.
