San Joaquin County Behavioral Health Services is seeking a qualified organization or organizations to provide Functional Family Therapy, a short-term, evidence-based family service delivered in community and home settings. Services will support children and youth ages 11–18 who have serious mental illness or co-occurring disorders. The solicitation package includes the RFP, Forms A through H, a sample Community Service Agreement, materials describing Functional Family Therapy implementation phases and training, a site-training and certification application, additional federal grant clauses, and instructions for the project budget, budget narrative, and timeline.
