The revised threshold-review checklist identifies the documents applicants must complete and upload with Homes for LA NOFA applications. Core materials include the completed UNOFA application and financial pro forma, conflicts-of-interest and legal/financial-liabilities self-certification, the self-score form, assurances and conditions form, and other program-specific forms. Depending on the applicable program and project, applicants may also need to provide title and site-control evidence, audited financial statements and a real-estate-owned schedule, appraisal and environmental reports, soils and asbestos or lead-based-paint documentation, architectural plans, relocation and replacement-unit materials, accessibility documentation, capital-needs assessments and scopes of work, preservation or stabilization plans, tenant-engagement materials, funding and financing commitments, evidence of entitlement readiness, project labor agreement materials, rent rolls, and other eligibility or scoring evidence. The checklist states that applicable items must be completed and submitted with the Homes for LA NOFA application.
