Description
Humboldt County seeks a qualified medical professional to provide ongoing non-emergency care to patients hospitalized at its Psychiatric Health Facility. Services include obtaining medical histories and performing physical examinations, providing basic treatment and medication consultations as needed, and advising County staff on patients' ongoing physical care. The provider may delegate services to appropriately licensed nurse practitioners and physician assistants, subject to direct supervision, quality monitoring, and peer-review responsibilities. Proposals must include a professional profile, project description, itemized cost proposal, supplemental documentation, references, evidence of insurability, business-license certification, and any exceptions or requested changes. The proposal must also include the County's Signature Affidavit, Cost Proposal Form, Supplemental Documentation, and Reference Data Sheet. The contract is anticipated to run from June 21, 2027, through June 30, 2029, subject to amendment or earlier termination.
