Description
Haywood County seeks a contractor to remove one existing Life Line multiplex Type I ambulance module from its current chassis, inspect and refurbish the module, and remount it onto one new Dodge Ram 4500 4x4 diesel chassis with an ambulance preparation package and approximately 168-inch cab-to-axle measurement. The scope includes structural repairs, exterior repainting and graphics, electrical and multiplex integration, HVAC and oxygen-system testing and repairs, warning and scene lighting, patient-compartment flooring, cabinetry, upholstery, seating, equipment transfer, and installation of specified chassis and ambulance components. The completed ambulance must be tested, calibrated, documented, warranted, and delivered clean, fueled, and service-ready. Bidders must provide detailed pricing, chassis information, compliance and exceptions documentation, remount methodology, allowances, delivery schedule, warranty and service-support information, references, subcontractor information, and a separate trade-in allowance for the original chassis. At least three similar-project references from the past five years are required, along with documented Type I ambulance remount experience and demonstrated Life Line multiplex capability. Addendum 1 states that build inspections may be conducted virtually or through build photographs, while the final factory inspection will be conducted in person with two county staff members attending. It also clarifies that taxes are excluded from the Bid Total, although bidders may omit taxes from the Bid Form; taxes apply only to invoices.
