Description
The City seeks licensed medical providers to deliver occupational health services for its police department. Services include illness and injury triage, return-to-work and fitness-for-duty evaluations, independent medical examinations, review of medical records, work-restriction recommendations, referrals and follow-up coordination, and medical examinations for gas-mask fitness. Telehealth is not accepted. Physical evaluations must be performed by a medical doctor, with a narrative report provided within five business days. The provider must also conduct observed 12-panel drug screenings using secure chain-of-custody procedures and provide blood alcohol testing. Scheduling must occur within 24 hours of notice; weekday operating hours are specified, and after-hours service is preferred. Proposals must describe the team, relevant experience, approach, professional capacity, and client references, including at least three references for similar services. Required response items include a cover letter, proposal details, financial disclosure, prior non-responsibility disclosure, workforce diversity form, M/WBE subcontracting draft plan, non-collusive certification, specified non-discrimination and data-sharing questionnaire responses, and a signed proposal confirmation. The contract duration is stated as “NO VALUE” from execution, with two one-year renewal options. Pricing is requested per examination or test for four service categories.
