Description
The Federal Bureau of Prisons is seeking a single qualified optometrist or optometry service provider to deliver routine clinical optometry care to incarcerated patients at FCI Jesup. Services include eye examinations, diagnosis of eye conditions, foreign-object removal, tonometry, written clinical reports, treatment and follow-up recommendations, corrective-lens prescriptions, eyeglass fitting and frame adjustments, and documentation in the Bureau of Prisons' electronic medical record. The contractor must provide one optometrist, maintain required Georgia licensure and malpractice and medical indemnification insurance, and furnish proof of the provider's education, training, qualifications, resume, and prior employment history. The work is an on-site, firm-fixed-price requirements-type service with one base year and four option years, for up to 384 one-hour sessions per performance period. The provider is expected to complete 20 to 25 scheduled examinations per day and attend a one-time orientation before beginning services. Contractor personnel must satisfy Public Trust and facility-security requirements, comply with privacy and correctional-facility rules, and submit pricing, references, a business management questionnaire, candidate qualifications, and insurance documentation. Award will be based on aggregate price, including options, and past performance.
