Milwaukee County Department of Health and Human Services, Children, Youth and Family Services, is seeking to add two contract agencies to its provider network for Children's Long-Term Support (CLTS) Waiver Program Support and Service Coordination. Services support Medicaid-eligible children and youth ages 0–22 with developmental disabilities, severe emotional disturbances, and/or physical disabilities. The provider will coordinate formal and informal supports, develop and update individualized service plans, monitor health, safety, outcomes, and service delivery, coordinate benefits and transitions, maintain required records, report incidents, and assist families with access to community, educational, medical, housing, vocational, and other resources. Providers must maintain qualified support and service coordination staff, supervision, case documentation, required participant contacts, training, background checks, and compliance with Wisconsin Medicaid waiver requirements. The solicitation states a billing rate of $26.78 per 15-minute unit, generally limits reimbursement to 2.5 hours per participant per month unless additional approval is obtained, prohibits subcontracting of the support and service coordination work, and requires billing through the ForwardHealth/Gainwell portal. Proposals are due November 11, 2026, at 4:00 p.m. CDT; the expected service implementation date is April 1, 2027.
