Description

Milwaukee County Department of Health and Human Services, Children, Youth & Family Services, is seeking approximately two new provider agencies to provide Children's Long-Term Support Waiver Program support and service coordination. The program serves eligible children and youth with developmental, physical, or mental health disabilities. Providers will coordinate eligibility and enrollment activities, functional-screen and recertification support, individualized service-plan development and monitoring, referrals and cross-system supports, participant and family contacts, documentation, incident response, and billing-related records. Agencies must comply with Wisconsin CLTS waiver requirements and ensure service coordinators complete required CLTS, mandated-reporter, and Children's Functional Screen training before serving participants. The Q&A states that agencies should plan for caseloads generally around 40–45 participants per coordinator, with some agencies potentially reaching 50 depending on capacity, and that the county currently has approximately 499 participants on its waitlist. Agencies may use their own case-note systems but must use the county's Avatar system for program-specific documentation.
