Description
Milwaukee County Department of Health and Human Services, Children, Youth & Family Services, is soliciting proposals from community agencies, organizations, joint ventures, and other interested parties to provide Children's Long-Term Support Waiver Program support and service coordination. The scope includes program support, service coordination, provider service-site information, staffing, accessibility, cultural competence, outcomes and quality assurance, and related agency and program documentation. Existing agencies do not need to reapply. The solicitation includes numerous required submission items, including a cover letter; organizational leadership and employee-demographic information; mission, vision, and values; conflict-of-interest, debarment, and disclosure certifications; targeted business enterprise certification; linked budget forms; agency and site information; licenses and certificates; program narrative; experience assessment; outcomes and quality assurance; cultural competence; accessibility; staffing plan and requirements; designation of confidential information; and a statement of deviations and exceptions. Services are expected to begin April 1, 2027.
