Description

Stony Brook University Hospital intends to procure nerve grafts, connectors, protectors, and related soft tissue items manufactured by Axogen Inc. This procurement will be conducted under NYS Education Law SS355(5)(A) discretionary authority, with a maximum value of $125,000. A sealed bid process will not be held. Interested vendors must contact the designated campus contact within 15 business days of the advertisement notice to submit a proposal. This procurement is subject to NYS Finance Law SS139 j&k concerning lobbying, and only the listed contact person is permitted for communication. Compliance with this law is crucial to avoid contract cancellation or debarment. The successful contractor will be required to submit documentation regarding Vendor Responsibility, NYS Finance Law 139, NYS Tax Law Section 5-a, and current NYS Workers' Compensation and Disability Forms. Proposals must be received by 3:30 PM on the listed due date. Axogen Inc. was awarded a substantially similar contract (SQ22-034) within the past five years.
