Description
The Commonwealth of Kentucky, through the Finance and Administration Cabinet and Kentucky Division of Oil and Gas, is soliciting a firm-fixed-price proposal to plug, remediate, and reclaim orphan well Permit No. 97124 in Magoffin County. The work includes mobilization and access-road preparation; H2S monitoring and safety controls; downhole tools and obstruction removal; cement plugging to surface; removal of wellhead equipment, flowlines, meters, separators, and other production infrastructure; handling of contaminated soil and potential TENORM; and restoration of the wellsite and access road using grading, seeding, drainage, erosion, and sedimentation controls. The proposal must include a signed solicitation face, signed latest addendum if applicable, cost proposal, technical solution, transmittal letter, required affidavit, Kentucky tax registration application, and foreign-entity certificate of authority documentation if applicable. The technical response is evaluated on personnel and project experience, equipment adequacy, work plan completeness, past performance, and cost. The selected contractor must plug and abandon the well within 30 days of contract award and submit the completed OG-38 plugging affidavit after site closure. A 5% bid guarantee is required, and the successful bidder must furnish separate 100% performance and payment bonds.
