Bid Form Certification: Bidder confirms site visit completed, scope fully understood, and all costs included. Signature, title, and date required on Bid Form. The following information needs to be provided on the Bid Form: Company Name, Address, City/State/Zip, Phone, Email, NC License #, Base Bid Total Lump Sum Price, Written Amount, Schedule, and Addenda information if applicable. Bidders must submit product data for all valves and major components either prior to their bid or with their bid. Allow at least two business days for approval of products submitted. If products submitted with a bid are rejected, the bid will be deemed non-responsive. The City reserves the right to reject any or all bids. All work shall be completed within 60 days of Notice to Proceed. Minimum three (3) year warranty on materials and workmanship. The warranty shall cover both materials and workmanship and shall not limit or replace any manufacturer warranty. Warranty shall begin upon final acceptance by the City. Bidders must submit: License information, Certificate of Insurance, Three references for similar work performed. Contractors shall comply with the requirements of NCGS 143-133.3 and shall certify that it and its subcontractors use E-Verify to verify the work authorization of employees. Contractor certifies that it is not listed on the Final Divestment List created by the North Carolina State Treasurer pursuant to NCGS 147-86.59. The City reserves the right to reject any bidder deemed not responsible based on past performance, safety record, or financial stability. Description
