Description
Texas Tech University Health Sciences Center (TTUHSC) is seeking proposals for implementation services related to their Collaborative Care Model (CoCM). The selected contractor will provide expertise to support the implementation, optimization, and scaling of the CoCM across three distinct health systems. Services include clinical, operational, and financial expertise to ensure model fidelity, sustainability, and high-quality patient outcomes. Key service requirements include providing clinical and operational technical assistance, such as developing workflow process maps, optimizing billing processes, analyzing patient funnels, and conducting training for healthcare providers. The contractor will also be responsible for delivering training materials, managing learning collaboratives, and potentially conducting site visits. The implementation will follow a phased approach: Pre-Implementation (Months 1-5), Launch (Months 6-7), and Post-Launch & Sustainability (Months 8-24).
