The County of Los Angeles, Executive Office of the Board of Supervisors is soliciting for a Work Order to provide as-needed consulting services to the Governance Reform Task Force. The goal of this engagement is to modernize and reform the current governance structure of Los Angeles County. This Request for Services (RFS) is being issued concurrently with a Request for Qualifications (RFQ) for a Consulting and Professional Services Master Agreement (CAPSMA). Vendors interested in applying for this RFS must also respond to the RFQ. Qualifying vendors will be issued a CAPSMA. To be considered, vendors are required to submit responses to both the RFQ and the RFS. Required documents for the RFQ include Vendor's Organization Questionnaire/Affidavit, Certification of Compliance, Debarment History and List of Terminated Contracts, Community Business Enterprise (CBE) Information, List of References, Contribution and Agent Declaration Form, Supplemental Financial Information (if applicable), and Declaration Financial Statements. For the RFS, required documents include the Response Cover Sheet, Response to Requested Information, References, and Staffing and Pricing Schedule.
