Description
Cortland County Health Department’s Tobacco Free Zone program is seeking community-based organizations to conduct community engagement, education, and mobilization activities addressing tobacco-related health disparities. Applicants may propose work involving smoke-free multi-unit housing, tobacco product waste, the retail tobacco environment, and/or tobacco-free worksites and outdoor properties in Cortland, Tompkins, and/or Chenango Counties. Activities include educating residents, officials, organizations, property owners, employers, and institutions; conducting community conversations or assessments; encouraging policy adoption; and providing in-person education to at least one receptive municipal body for applicable initiatives. Lobbying is prohibited, and subaward recipients must maintain a written 100% tobacco-free policy. Up to three recipients will share $35,000. Activities must occur from December 1, 2026, through April 30, 2027, with funds spent by April 30, 2027; a final report is due by May 15, 2027. Proposals must use the provided template and include an itemized budget, project scope and timeline, organizational experience and capacity, evaluation and reporting plans, applicable licenses and certifications, references, litigation information, W-9, insurance proof, and required exhibits and qualification questionnaire. Separate proposal documents are required for applicants seeking funding for more than one initiative.
