Description
The City seeks a qualified audiologist or audiology firm to administer a hearing protection program for employees with occupational noise exposure. Services include on-site audiology testing and professional hearing examinations, retesting and retraining for employees with a standard threshold shift, and coordination with the City's Safety Program Manager. The City estimates 350–400 employees may participate, with testing typically scheduled across 15–20 time slots of four to eight hours each. The provider must prepare examination reports within 48 hours, deliver or transmit the original report within 10 business days, and maintain employee records for one year while providing a copy or electronic record to the City. Proposals must include an approach and qualifications, personnel information and resumes, proposed fees, at least three references for projects completed within the last two years, business-name information, a business-license commitment, and the proposal's validity period. The selected provider must be properly licensed in Washington. The initial agreement is two years, with three optional two-year renewals.
