Description
The Department of Veterans Affairs is soliciting one firm-fixed-price, indefinite-delivery requirements contract for Tacrolimus capsules in 0.5 mg, 1 mg, and 5 mg strengths, packaged in 100-count bottles. Estimated annual usage is 51,904 bottles of 0.5 mg capsules, 142,788 bottles of 1 mg capsules, and 5,207 bottles of 5 mg capsules. One award will be made in the aggregate for all three line items, including the base year and four one-year options; offerors must price every line item for every contract period and include the 0.5% cost recovery fee. Offerors must provide unique 11-digit NDC numbers for each product, identify the exact product name and strength appearing on the label, and meet FDA approval and current good manufacturing practice requirements for all applicable manufacturing, packaging, and other facilities. Products must meet specified pharmaceutical packaging, barcode, Drug Supply Chain Security Act, and country-of-origin requirements. Distributor offerors must submit a manufacturer Letter of Commitment assuring an uninterrupted source of supply. Products will be distributed through VA and DoD Pharmaceutical Prime Vendor Programs to authorized VA, DoD, Bureau of Prisons, Indian Health Service, Federal Health Care Center, and eligible State Veterans Homes participants.
