Description
Mohawk Medbuy Corporation, on behalf of Michael Garron Hospital/Toronto East Health Network, is seeking one or more proponents for two benefit categories: Category 1, Accidental Death and Dismemberment (AD&D) Insurance; and Category 2, Life and Long-Term Disability (LTD) Insurance, Health and Dental Benefits. The categories may be awarded independently to the same or different proponents. Services include matching or improving existing plan designs, providing insurance coverage, administering insured and administrative-services-only health and dental claims, coordinating benefits, supporting transition and lag claims, and providing required reporting. Proponents must complete the applicable pricing templates, questionnaires, plan-design and deviation schedules, mandatory requirements, reference information, conflict-of-interest declarations, value-added benefits schedule, and other required portal submissions. The anticipated agreement term is five years, with up to two additional one-year extension options. Pricing must be submitted in Canadian dollars, and the proposal must be submitted electronically through the designated procurement portal.
