Description
Passaic County Department of Human Services seeks a consultant or qualified organization to provide individualized case consultation and navigation for county residents with intellectual and developmental disabilities, including teenagers and young adults with severe learning disabilities, Down syndrome, autism-spectrum disorder, and traumatic brain injury. Services include helping individuals and families identify, apply for, and obtain appropriate resources; coordinating with state and local agencies; addressing financial, transportation, and language barriers; collaborating with community partners and the state ombudsman; and providing community education, outreach, and presentations. The consultant must work on-site at the County Department of Human Services for at least 15 hours per week, develop a screening tool and train department staff, maintain records at the department, meet biweekly with department leadership, and create marketing and educational materials. Estimated funding is $45,000 for the initial period through December 31, 2026, followed by estimated annual funding of $60,000 for up to four one-year options. The proposal must include the specified bid forms, Annex A materials, program narrative, outputs, budget, agreement, organizational information, and supporting memoranda of understanding.
