Description
The Town of Flower Mound is accepting Statements of Qualifications from qualified firms to establish a list of approved firms for subsurface utility engineering and locating services for various projects. Services must meet Quality Levels A and/or B under CI/ASCE 38, and consultants must employ professional engineers able to stamp plans when applicable. Potential service areas include Level A SUE, Level B SUE, pavement removal and replacement, and non-invasive ground-penetrating radar. Addendum No. 2 clarifies that pavement removal and replacement is limited to restoration of pavement disturbed by SUE activities, such as coring, potholing, and test holes. Firms may submit qualifications for only one service category and do not need to be proficient in every listed area. Selected firms will be retained for two years and may subsequently be requested to submit project-specific service proposals for approval. Responses must address qualifications and relevant project experience, references, time in business, quality assurance, prior work for the Town if applicable, project manager and key personnel backgrounds, team roles, proposed services, timely performance capability, and disclosure of significant contract failures or pending litigation or investigations. The response must include the Town of Flower Mound Proposal Certification Form, a completed and signed Conflict of Interest Questionnaire, a current Certificate of Insurance, and an SOQ. Responses must be submitted electronically in PDF format through the electronic bidding system.
