Description
The Defense Logistics Agency is soliciting a quotation for one disposable, sterile minor procedure instrument kit. The five-piece kit must contain one straight Crile hemostat, one straight iris scissors, one medium McGill, one Hagar-Mayo needle driver, and one combat cravat. Components must be packaged in a 6-mil nylon-poly barrier bag, vacuum sealed, and gamma radiated, with a 36-month shelf life. No more than five months may elapse between manufacture and delivery. The item is FDA-regulated and must meet applicable registration, listing, labeling, and good manufacturing practice requirements. The quotation must identify the source and part number being supplied; alternate offers require complete technical data. Delivery is FOB destination, with destination inspection and acceptance, ASTM D3951 packaging unless superseded by DLA requirements, MIL-STD-129 marking, and shipment by traceable means. The solicitation is not a small-business set-aside, and quotations must be submitted through the DLA Internet Bid Board System.
