Description
The Maryland Department of Health is seeking an eligible nonprofit organization, local or state government agency, public college, or state university to develop or enhance a comprehensive transition program for young adults with sickle cell disease entering adult-centered care. The program must address relationships among patients, parents, and providers; client expectations and goals; psychosocial needs; developmental maturity; health literacy; family functioning; self-management; neurocognitive considerations; and provider-patient communication. The selected applicant must integrate a Transition Coordinator capable of assessing psychosocial needs, navigating complex medical-legal systems, and conducting crisis interventions. The work includes program planning, evidence-informed evaluation tools and action-plan templates, administrative protocols and workflows, referral coordination, staffing and training, quarterly reporting, and coordination with existing Community Health Worker and Hub-and-Spoke programs without duplicating their services. The agreement is cost reimbursable and is expected to include a base period from October 1, 2026, through June 30, 2027, plus two option years.
