Description
Ulster County Department of Social Services is seeking one qualified agency to provide intensive, short-term, in-home and in-person case management and family preservation services for families with open Foster Care or Child Protective Services preventive cases. Services may be provided in homes and other community-based settings for periods of up to six weeks and should focus on family stabilization, child safety, family functioning, parenting support, crisis intervention, access to community resources, and preventing out-of-home placement when feasible. Proposals should address culturally responsive, trauma-informed, strength-based, family-centered, evidence-based or evidence-informed services; staffing and implementation; measurable outcomes and evaluation; data collection; follow-up care after foster-care reunification; and sample reports. The submission must include a technical proposal addressing qualifications, experience, organizational capacity, proposed service plan, implementation timeline, staffing, resumes, organizational chart, deliverables, outcomes, evaluation methods, references, and reports. A separate fee/cost proposal and detailed line-item budget and narrative are required. The solicitation also requires numerous certifications and return forms, including ownership disclosure, living-wage acknowledgment, non-collusion affidavit, Iranian divestment certification, MacBride Fair Employment Principles acknowledgment, insurance acknowledgment, and addenda acknowledgment if applicable. The contract term is one year with two additional one-year renewal options.
