Description
The Department of Veterans Affairs is seeking a licensed Nuclear Regulatory Commission vendor to provide Fludeoxyglucose F18 (FDG) radiopharmaceutical unit doses for the Nuclear Medicine Department. The estimated quantity is 1,000 units for each of one base period and four option periods, ordered as needed, with delivery to the secured Nuclear Medicine receiving area. Routine orders require next-business-day delivery by 8:00 a.m.; priority orders must be delivered within two hours, with priority orders estimated at five per year. Each radioactive shipment must be properly labeled, and each unit dose must include the radiopharmaceutical name, amount, preparation time, expiration time, date, and a bar-coded insert compatible with the nuclear medicine information management system. The contractor must provide system migration, technical support, and staff training at no additional cost if its system differs from the current system. The contractor must accept qualifying unused or damaged doses for full credit toward future orders and provide delivery receipts and monthly invoices. Quotes must include technical capability information, product brochures or equivalent materials, OEM name, part number, country of origin, line-item pricing, up to three similar past-performance references, an OEM authorization letter when the offeror is not the OEM, and a completed Standard Form 1449.
