Description
Sherburne County Health and Human Services is seeking one or more independent, licensed Minnesota professional guardianship-conservatorship agencies to provide services to county residents throughout the county. The selected provider(s) will charge a monthly flat rate per active client and must provide properly credentialed staff and volunteers, coverage hours, client-related recordkeeping, reporting, billing, and services consistent with applicable statutes. The contract term is 36 months beginning January 1, 2027, with annual reviews and potential renewal subject to available funding and successful progress. Proposals must include a cover letter and a narrative of no more than five pages addressing organizational qualifications, relevant experience, challenging-behavior experience, recordkeeping, monthly flat rates, no-show policy, contact frequency, staffing ratios, and coverage hours. Respondents must carry general liability insurance of $1,500,000 per claimant and $1,500,000 for claims arising from a single occurrence, and must be independent and licensed to practice in Minnesota.
