Milwaukee County Department of Health and Human Services, Area Agency on Aging, is seeking one qualified provider for a 2027 program serving LGBT adults age 60 and older who are experiencing significant economic and/or social need. The provider must offer culturally relevant social and recreational activities, one-on-one support and referrals to community and county services, advocacy opportunities, community education, outreach, recordkeeping, and program data collection and reporting. The program must serve at least 80 unduplicated older adults through group programming, provide at least 20 one-on-one service connections or referrals, participate in four participant-recruitment outreach events, and participate in four events promoting awareness of LGBT older adults’ needs. The provider must track outcomes, submit monthly reports and invoices, conduct an annual client satisfaction survey, and present outcomes at least twice annually. The anticipated 2027 funding is $30,000, with a 10% local match requirement for the Title III B portion; reimbursement will be based on actual allowable expenses under a grant contract. The award period is January 1 through December 31, 2027, with four possible one-year extensions.
