This Request for Proposal seeks construction services to remove and dispose of the existing undersized culvert at South Fork Running Creek and install a new 44-foot-6-inch spanning steel bridge beneath USFS Road 357. The work includes bridge excavation and construction, five rock weirs, one channel barb, stream and bank stabilization, erosion and sediment controls, seeding, roadway embankment and aggregate work, structural steel and timber components, and related site restoration. The contractor must also improve three Horseshoe Bend corners on USFS Road 357 to accommodate bridge-material deliveries and general forest traffic, with temporary traffic control and road closures coordinated with the relevant parties. The contractor must provide labor, materials, equipment, qualified operators, permits, environmental controls, and project management. In-stream work is restricted to July 15 through August 15, 2027; construction may begin June 1, 2027, and all work must be completed by October 15, 2027. The response must include a signed proposal cover sheet, three lump-sum cost schedules, a 5% proposal deposit in the form of a bid bond or certified/cashier's check, and a proposal response addressing at least three references, company and key-personnel qualifications, relevant project experience, and the proposed construction approach, schedule, staging, sequencing, and equipment. The proposal response is limited to 12 pages. Performance and labor-and-material payment bonds, each equal to 100% of the contract price, are required after award. The contractor must be licensed in Idaho and maintain the specified general liability, errors and omissions, automobile liability, and workers' compensation insurance.
