Description
The Defense Logistics Agency is seeking a quotation for one package of endotracheal tube holders, NSN 6515-01-699-2332. The requested item is an endotracheal tube holder with a 6.5 mm inner diameter to 21 mm outer diameter, a screw mechanism to secure the clamp, a large opening for viewing the patient's mouth and lip color, and bite and pinch prevention features. The unit of issue is a package containing 25 each. The item must have a 36-month shelf life, with no more than five months elapsed between the manufacturer's date and delivery to the Government. The quotation must identify the source and part number offered. Alternate offers require complete technical data sufficient for evaluation. The item is an FDA-regulated Class I medical device and must comply with applicable registration, listing, labeling, good manufacturing practice, packaging, marking, domestic sourcing, and inspection requirements. Delivery is required FOB destination, with commercial packaging and medical marking requirements specified in the solicitation.
