Description
The USDA Forest Service seeks licensed medical professionals to provide basic healthcare for students at the Flatwoods Job Corps Center. Services include examinations and assessments, treatment, prescriptions and immunizations, monitoring health conditions, referrals and follow-up care, health promotion, prevention, and related coordination with center staff. Services must be provided on site, generally one or two days per week, with the schedule coordinated with center personnel. The quotation must include the completed Quoter’s Questionnaire and Schedule of Services with pricing for the base year, four option years, and possible six-month extension. It must also include proposed key personnel’s resumes, unrestricted state medical licenses, DEA registrations, and proof of medical liability insurance, along with the requested company information, representations, and three professional references for similar past work. The contract is a one-year base period with four one-year options and a possible six-month extension; the solicitation states that the total duration may not exceed five years and six months.
