Description
The City of Lake View, Alabama, seeks Statements of Qualifications from professional firms to provide post-award material testing and special inspection services for a stand-alone, site-built community tornado safe room designed to meet FEMA Publication 361 and ICC-500 criteria for 250-mph winds. The scope may include soil compaction testing; verification of footer depth, width, and compaction; rebar placement and ties; concrete-pour testing for footers, the building pad, block walls, and roof systems; and review, reporting, and certification based on architectural, engineering, civil, MEP, and structural plans and specifications. Respondents must demonstrate at least three recently completed projects of the indicated type, firm and proposed-personnel qualifications, relevant FEMA 361 and ICC-500 experience, capacity to perform the work, conflict-of-interest disclosure, enrollment in the U.S. Department of Homeland Security's E-Verify program, and Alabama licensure. The SOQ is limited to four pages, excluding E-Verify and licensure attachments, and must be signed by an authorized representative. The selected firm will work with the general contractor and project stakeholders and enter into a contract based on a negotiated not-to-exceed amount. Proof of specified insurance is required before contract execution.
