University Health Network is soliciting a supplier to provide and deliver hemodialysis chairs. The chairs must support the needs of hemodialysis patients and staff, with emphasis on quality, durability, comfort, optimal features, patient and staff satisfaction, and improved ergonomics. The submission must include a signed declaration form, a pricing file, and completed response templates for the Hemodialysis Chairs General Requirements questionnaire and Hemodialysis Chairs Environmental Exhibit questionnaire.
