Description
Apache Elementary School is seeking quotes for Category One internet access and data transmission service at 300, 400, and 500 Mbps, with a quantity of one service connection. The requested service includes installation and initial configuration, must allow upgrades to the quoted bandwidth, and must remain price-valid for the life of the contract. Quotes must include all applicable fees and Arizona sales tax, separately itemize ineligible costs, display the provider's SPIN and FCC Form 470 number 270002135, and offer the applicant SPI or BEAR invoicing options. Proposed services and equipment must comply with applicable FCC Lowest Corresponding Price requirements and restrictions concerning Huawei, ZTE, and other covered companies. This is not a formal sealed bid or RFP process; contracts under 60 months should provide for annual or month-to-month extensions, not exceeding 60 months.
