Description
Fort Bend County seeks one or more vendors to supply medical oxygen to its EMS locations, including cylinders and related labor, materials, equipment, chemicals, and personal protective equipment. The scope includes next-day delivery for oxygen orders, pickup and delivery for hydrostatic testing with a two-day turnaround, and closed-loop RFID tracking of cylinders. Cylinders must contain medical oxygen only, be tagged as full, in use, or empty, and be labeled with a lot or batch number, expiration date, and purity test result of at least 99.5%. Pricing must be all-inclusive, with no additional fees permitted. The contract runs through September 30, 2027, with annual renewals for up to four years through September 30, 2031, subject to mutual agreement. Respondents must submit a current certificate of insurance or an eligible notarized insurance-company statement with their response. The bid also requires a completed vendor form, W-9, tax/debt/residence certification, pricing form, and contract sheet. The solicitation states that Form 1295 is due from apparent low bidders within three calendar days of notification; publicly traded companies are exempt.
