Description
The City of Alhambra is soliciting proposals from qualified occupational health providers to deliver as-needed services for approximately 380 full-time and 225 part-time municipal employees. The scope includes pre-employment, return-to-work, fitness-for-duty, DOT, firefighter, police, and other medical examinations; drug and alcohol testing; vaccinations; tuberculosis, respiratory, audiometry, vision, and post-exposure screenings; occupational injury treatment and workers' compensation case management; physical and occupational therapy; functional capacity and range-of-motion services; and related medical consultation and reporting. Services must be performed at the provider's facilities, with the primary clinic located within 20 miles of Alhambra, California. Providers must include at least two physicians on the National Registry of Certified Medical Examiners, at least two breath alcohol technician-certified staff, certified testing facilities, secure electronic reporting, confidential record maintenance, and the ability to schedule return-to-work examinations within two business days. The City has historically used approximately 15 cardio stress tests annually, rapid urine testing without THC for non-DOT staff, and laboratory-based testing through Quest Diagnostics for DOT staff. National Safety Compliance, Inc. serves as the City's DOT third-party administrator and manages random-pool sampling, laboratory coordination, and reporting. Proposals must include organizational qualifications, relevant experience, references, work plan, reporting approach, scope narrative, contract-performance history, sample reports, and a detailed schedule of charges. The selected provider must obtain a City business license and maintain specified liability, professional, automobile, workers' compensation, employer's liability, and medical malpractice insurance.
