The Indiana Department of Natural Resources is soliciting a contractor to provide a two-year full-maintenance program for elevator equipment at Pokagon State Park's Potawatomi Inn. The scope includes scheduled inspections and preventive maintenance, cleaning, lubrication, adjustments, repairs, replacement of covered parts, callback service, emergency response, safety testing, maintenance documentation, and compliance with manufacturer requirements, ANSI A17.1, and applicable codes. The equipment inventory includes three Otis elevators, with related vertical-transportation equipment and appurtenant systems covered as specified. The contractor must perform twelve scheduled service visits per year, maintain required parts and records, provide maintenance-control documentation and safety-test reports, and respond to emergency calls by phone within 30 minutes with onsite service within a timeframe agreed with the property manager. The bid package requires the completed state quotation/bid form and a sealed submission; the contractor must self-perform at least 15% of the work and may subcontract no more than 85%.
