Description
The Village of Melrose is soliciting sealed bids for repair of a failing culvert on SR 613 north of the railroad tracks. The work includes inserting and grouting a 54-inch liner pipe, improving the inlet, repairing the embankment, restoring excavated areas, replacing the existing guardrail and end treatments, coordinating traffic control, removing construction debris, and restoring the site. All labor and materials must comply with applicable Ohio Department of Transportation specifications, domestic steel requirements, permits, and prevailing-wage requirements. The contractor must provide all labor, materials, equipment, and related services; coordinate utilities and required agencies; attend a pre-construction meeting; obtain a Notice to Proceed before beginning work; and complete the project by April 15, 2027. The bid must include the required bid proposal forms, bid guaranty, contract bond, affidavits, subcontractor list, addenda receipt, and applicable insurance documentation. A bid bond equal to 10% of the bid is required, and the contractor must maintain specified general liability, workers’ compensation, employers’ liability, and automobile liability coverage.
