Description
Salt Lake City School District is requesting Statements of Qualifications from vendors that can provide food items or other products for fundraising activities at district schools. Selected vendors will be placed on an open-ended approved vendor list for 18 months, and schools may select approved vendors as needed. Vendors must specify either a percentage of profits or a flat fee per item sold, with proceeds going exclusively to the district. Responses must address the fundraiser's operation, school profit structure and return calculation, vendor qualifications and experience, financial stability, at least three recent client references, and complaint or defective-merchandise resolution procedures. Required supporting materials include a Utah Department of Commerce fundraiser permit, commercial co-venturer letter, or exemption letter; a valid Utah business license; a current IRS Form W-9; an insurance certificate; and any vendor-proposed terms or contract documents. The submission must also acknowledge and accept the district's limitations, FERPA terms, general provisions, and other solicitation requirements.
